29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about March 31, 2021, at a construction site, three employees were exposed to struck-by and crushed-by hazards while placing a water line in an excavation with a depth exceeding more than 5 feet without adequate protection from cave-ins. Blazey Construction Services, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1451086, citation number 1, item number 3, and was affirmed as a final order on February 6, 2020, with respect to a workplace located at the Intersection of FM 2920 and Grand Parkway in Tomball, TX, Tomball, TX 77375.
