29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Warehouse- Area: On or about 03/12/2021, the employer exposed employees to fire hazards in that an APC UL 120V ac relocatable power tap and extension cord were daisy-chained and installed for use with a Phoenix Wrap Machine (Model# PLPA-2150, Serial# 19055147, Single Phase 110V).
