29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a. On or about February 3, 2021, at the site located on SR 528 mile marker 11.7, employees were exposed to cave in hazards when the employer failed to install protection for employees who were installing a man-hole box in an excavation of approximately 8 feet in width x 10 feet depth. b.
