29 CFR 1910.134(m)(2)(i)(C): The employer did not establish a record of the qualitative and quantitative fit tests administered to an employee which included the specific make, model, style and size of respirator tested: a) ICU and IMCU, Atlantic Health System dba Chilton Medical Center, 97 West Parkway, Pompton Plains, NJ 07444. The employer provided and required employees including, but not limited to Registered Nurses to wear N-95 respirators. Employee fit testing records did not include the model and/or size of N-95 respirator. Violation occurred on or about 2/18/2021 and thereafter.
