29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident a) Hamilton 2 and Madison 2: An employee working as an Assistant Director of Nursing died of occupational exposure to COVID-19. The employer failed to report the death of an employee to OSHA within 8 hours of learning about the fatality. The violation occurred on or about 4/18/2020. b) Hamilton 2 and Madison 2: An employee working as a certified Nursing Assistant (CNA) died of occupational exposure to COVID-19. The employer failed to report the work-related death of an employee to OSHA within 8 hours of learning about the fatality. The violation occurred on or about 4/16/2020.
