Text For Citation: 02 Item/Group: 002 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (1) authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. (2) turn off and shut down the machine (3) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources. (4) affix lockout or tagout devices to each energy isolating device (5) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished. a) On 5/14/2020 an employee was in the process of clearing a jam from a PACMAC Bagging machine (model # 9500). The employee laid down on the ground to reach up inside the machine with the right hand trying to dislodge the jammed bag, while engaging the activation button with the left hand. As a result the employee right hand was caught in the blade for the bagger. The company did not ensure that 29 CFR 1910.147(d)(1),(2),(3),(4) and (d)(5) were followed prior to reaching into the PACMAC. JCG Industries, Inc. DBA Koch Foods Poultry, Inc. was previously cited for a violation of this Occupational Safety and Health standard (1910.147 (d), which was contained in OSHA inspection number 1073993, citation number 01, item number 02 and was affirmed as a final order on 1/7/2016, with respect to a workplace located at 4404 W Berteau Ave. Chicago, IL. 60641 JCG Industries, Inc. dba Koch Foods, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1210204, citation number 2, item number 1 and was affirmed as a final order on June 28, 2017, with respect to a workplace located at 4404 W. Berteau Ave. Chicago, IL 60641.
