29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of a warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitor system. Location - 205 Newbury Street, Framingham, MA, 01701. On or about April 11, 2020, fourteen employees were working on the roof near unprotected skylights while installing a new roof. The employees used a warning line system and a safety monitor was properly designated by the employer. One of the employees was working outside the warning line near the edge of the roof at a height greater than 40 feet above lower level and fell off to lower level. The RCH Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(4)(ii), which was contained in OSHA inspection number 1403544, Citation number 1, Item number 1, and was affirmed as a final order on July 10, 2019, with respect to a workplace located at 334 Russell Street, Hadley, MA.
