29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition. a) Car Yard: On or about February 24, 2020, employees were exposed to struck-by hazards while operating a Forklift (TCM Model FG25 Serial Number 2781991), in that the forklift had a frayed hydraulic hose located at the mast but was not taken out of service. b) Car Yard: On or about February 24, 2020, employees were exposed to struck-by hazards while operating a Forklift (TCM Model FG25 Serial Number 2781991), in that the forklift had worn tires that could affect its balance but was not taken out of service.
