29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about 2/20/2020, at the construction site located at 31 Venetian Way, Miami Beach, FL 33139. 4th floor, Building B - An employee was exposed to an electrical shock hazard while energizing a portable power tool using an extension cord that lacked a ground prong on its attachment plug.
