29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, was not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage and equipment found damaged or defective was not removed from use until repaired: (a) On or about February 6, 2020 and at times prior, for the employee exposed to electrical shock hazards associated with a temporary electrical cord sets which were damaged exposing inner wiring at or near 224 Second Avenue West in Ray, North Dakota.
