29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process. At a worksite located at 17562 Gum Rd, Carthage, MO: Employees were exposed to fire and explosion hazards. The employer failed to protect employees in that the Piping and Instrumentation Diagram (PIDC-CPPR-3148-11) developed for the system is not accurate, complete, and up-to-date for the covered process equipment associated with the synthesis of various chemical products using AN and SN prills, K37, and 7019 as the reagents associated with the Chub Dry Ingredient Transfer process equipment. a) P&ID was not accurate for as a hopper labeled FD-002 was misidentified when the legend identifies that same hopper as F-002.
