29 CFR 1904.29(b)(2): The employer did not complete an OSHA 301 Incident Report form, or an equivalent form, for each recordable injury or illness entered on the OSHA 300 Log: Carcoustics USA South Inc., Buford, GA - On December 12/10/19, an employee experienced a reaction to dry ice that required days away from work. An incident report form was not completed as required by the standard, on or about 1/21/20.
