29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Ball, Bounce and Sport, Inc. - Dunkirk, Ohio: On or about December 20, 2019, employees were exposed to rotating parts, pinch points, and struck-by hazards while cleaning equipment such as, but not limited to, the polyethylene mixer and 220 mold machine. The employer failed to develop and document procedures that would control hazard energy sources of the equipment. Ball, Bounce and Sport, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1374595, citation number 2, item number 2 and was affirmed as a final order on April 26, 2019, with respect to a workplace located at 1860 Baney Rd., Ashland, Ohio, 44805.
