29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source. a. On or about December 26, 2019, in the bucking unit area of the thread shop where employees were exposed to caught-in and struck-by hazards while performing maintenance and servicing work on a bucking unit on line #1 without isolating the machines energy sources. b. On or about December 26, 2019 and at times prior thereto, in the bucking unit area of the thread shop where employees were exposed to caught-in and struck-by hazards while performing maintenance and servicing work on a bucking unit on line #2 without isolating the machines energy sources. Pinnacle Machine Works, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(d)(3), which was contained in OSHA inspection number 1253288, citation number 1, item number 2 and was affirmed as a final order on September 21, 2017 with respect to a workplace located at 10521 Sheldon Road, Houston, TX 77044.
