29 CFR 1904.39(a)(2): The employer did not, within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, report the in-patient hospitalization, amputation, or loss of an eye to OSHA: a) Job site, 33 McGovern Road, Crescent, PA: On or about November 23, 2019, the employer did not report an amputation injury to OSHA within the required 24 hour period. The injury occurred on Saturday, November 23, 2019 at approximately 10:20 AM and it was not reported to OSHA until Sunday, November 24, 2019 at approximately 3:23 PM.
