29 CFR 1910.147(c)(5)(ii)(B): Lockout and tagout devices were not standardized within the facility in at least one of the following criteria: color; shape; or size: a. Site- The employer did not ensure that lockout devices were standardized where locks used in drive room #1 to lock out a 2400 volt fused disconnect utilized both an unpainted steel lock and a painted lock; and one lock identified an employee by their full name while the other lock identified an employee by their nick name. b. Site- The employer did not ensure that tagout devices were standardized where tags used in drive rooms #1 and #2 to tag out a 480 volt and a 2400 volt fused disconnect utilized a yellow tag with no employee name on the tag and a red and white danger tag with an employees name on the tag.
