29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): (REFERENCE 1926.602(d): a) jobsite, 949 N Hermitage Rd. in Hermitage, PA. On or about November 20, 2019, a JLG all-terrain forklift was operated by employees to lift shingles and roofing materials to the roof. A record of forklift operator training was not certified to verify that each employee had received training to operate the forklift. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
