29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: On or about November 1, 2019, the employer failed to implement permit confined space procedures when employees are required to enter permit required confined spaces for spray painting and sand blasting operations inside holding tanks. Employees exposed to inhalation hazards, fire hazard from flammable paints igniting and falls from opening in the floor and rotating tank. Alexander Tank Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.146 (c)(4), which was contained in OSHA inspection number 1291102, citation number 1, item number 9a and was affirmed as a final order on August 24, 2018, with respect to a workplace located at 2400 E Pierce Street, Luling TX, 78648.
