29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: (a) On or about October 03, 2019 - Bagging area, employees were exposed to struck-by hazards when the electrical verification step said to attempt to start the equipment by activating the equipment operator controls (On & Off) button/switch. The Pacmac does not have an On/Off button or switch. Pneumatic said to bleed residual air.
