29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: a. 1000 Riverside Dr., Keasbey, NJ - Warehouse The employer did not provide written certification of training of employees authorized to operate powered industrial equipment. Employees engaged in the warehousing of operated powered industrial trucks, which included, but was not limited to, Yale 50VX forklifts, Yale 60 VX forklifts, and Komatsu Clamp forklifts, were not certified by the employer prior to being authorized to operate the equipment.
