29 CFR 1910.272(g)(1)(ii): Prior to entry, all mechanical, electrical, hydraulic, and pneumatic equipment which presented a danger to employees inside grain storage structures in the grain handling facility were not disconnected, locked out and tagged, blocked-off, or otherwise prevented from operating by other equally means or methods: (a) GRAIN BIN 12 - Employees emptied and cleaned grain bin 12, and employee(s) could not physically perform lock out tag out while installing and disconnecting the equipment because there was no physical way to secure the electrical equipment in a de-energized position, on or about June 1, 2019. (b) GRAIN BIN 5 - Employees emptied and cleaned grain bin 5, and employee(s) could not physically perform lock out tag out while installing and disconnecting the equipment because there was no physical way to secure the electrical equipment in a de-energized position, on or about June 1, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records. Risser Grain LLC, was previously cited for a violation of this occupational safety and health standard 1910.272(g)(1)(ii), which was contained in OSHA inspection number 1189245, citation number 1, item number 2 and was affirmed as a final order on March 17, 2017, with respect to a workplace located at 1196 Holtwood Road, Holtwood, PA 17532.
