29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedures and requirements of this standard were being followed: a. 208 Cougar Court, Hillsborough, NJ - Production Area The employer did not perform periodic inspections of the LOTO procedures. Employees engaged in the maintenance and servicing of equipment that included, but was not limited to Tri-Delta and Jomar injection blow molding machines, used energy control procedures on a daily basis that were not evaluated at least annually, to ensure the lockout/tagout procedures were valid, identified all sources of hazardous energy, and employees were utilizing the procedures. Violation occurred on or about July 10. 2019 through at least July 18, 2019 b. 210 Cougar Court, Hillsborough, NJ - Warehouse and Grinding Room The employer did not perform periodic inspections of the LOTO procedures. Employees engaged in the maintenance and servicing of equipment that included, but was not limited to Allan-Bradley and Ball & Jewel grinding machines, used energy control procedures on a daily basis that were not evaluated at least annually to ensure the lockout/tagout procedures were valid, identified all sources of hazardous energy, and employees were utilizing the procedures. Violation occurred on or about July 10. 2019 through at least July 18, 2019 c. 412 Ridgefield Rd., Hillsborough, NJ - Production Area The employer did not perform periodic inspections of the LOTO procedures. Employees engaged in the maintenance and servicing of equipment that included, but was not limited to Milicron and Jomar injection blow molding machines, used energy control procedures on a daily basis that were not evaluated at least annually to ensure the lockout/tagout procedures were valid, identified all sources of hazardous energy, and employees were utilizing the procedures. Violation occurred on or about July 10. 2019 through at least July 18, 2019 d. 420 Ridgefield Rd., Hillsborough, NJ - Production Area (PET) The employer did not perform periodic inspections of the LOTO procedures. Employees engaged in the maintenance and servicing of equipment that included, but was not limited to Nissei ASB injection blow molding machines, used energy control procedures on a daily basis that were not evaluated at least annually to ensure the lockout/tagout procedures were valid, identified all sources of hazardous energy, and employees were utilizing the procedures. Violation occurred on or about July 10. 2019 through at least July 18, 2019 Pretium Packaging, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1292088 citation number 1, item number 2a, and was affirmed as a final order on May 8, 2018, with respect to a workplace located at 2015 S. Main Street, Paris, IL 61944 Pretium Packaging, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1044386 citation number 1, item number 2c, and was affirmed as a final order on March 26, 2015, with respect to a workplace located at 200 W. 20th Street, Hermann, MO 65041.
