29 CFR 1910.147(d)(4)(ii): Lockout devices, where used, shall be affixed in a manner to that will hold the energy isolating devices in a "safe" or "off" position. On or about May 17, 2019 the employer failed to ensure that the energy isolating devices of the Four-Head (350R2) blow mold rotogranulator were operatied in a manner to isolate the hazardous energies of the equipment prior to performing maintenance. Hazardous energies include mechanical and electrical. Superior Dairy was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1296891, citation number 1, item number 1 and was affirmed as a final order on May 19, 2018 with respect to a workplace located at 4719 Navarre Road, Canton, OH 44706. Superior Dairy was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1296897, citation number 1, item number 1b and was affirmed as a final order on May 19, 2018 with respect to a workplace located at 4719 Navarre Road, Canton, OH 44706
