29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a) Jobsite: 2245-47 Wilder Street, Philadelphia, PA 19146 - Employees conducting masonry operations from supported tubular welded frame scaffolds were exposed to fall hazards, between approximately 13 feet and 26 feet above ground level, without the use of a fall protection system. Noted on or about 4/12/19. O'Neill Masonry, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1018867, citation number 1, item number 1a, and was affirmed as a final order on 6/2/15, with respect to a workplace located at 1613 S 21st St, Philadelphia, PA.
