29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) 1404 East Palmer St., Philadelphia, PA 19125 - The employer did not ensure that the working levels of a fabricated frame scaffold were fully planked while employees performed brick masonry on them at heights of up to approximately 24 feet, thus exposing employees to fall from height hazards. This condition was observed on or about April 9, 2019. Vyacheslav Leshko aka Slavik Leshko was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1218454, citation number 2, item number 2 and was affirmed as a final order on October 11, 2017, with respect to a workplace located at 827 North 26th Street, Philadelphia, PA 19130.
