29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident On or about March 20, 2019, an employee sustained a fatal injury from a high-pressure hose nozzle that disconnected and struck him in the head. The employer did not report the fatality to the Occupational Safety and Health Administration.
