29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a. On or about February 6, 2019, at the job site, the roof access ladder used by 5 roofing employees at the left eave was extended only 14 inches above the edge of the eave, exposing the employee to an 8 feet fall hazard from the top of the ladder. b. On or about February 6, 2019, at the job site, the roof access ladder used by the Crew Leader was extended only 22 inches above the roof edge above the front entrance, exposing the Crew Leader to an 8 feet fall hazard from the top of the ladder. Travis Slaughter d/b/a/ Great White construction, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.1053(b)(13), which was contained in OSHA inspection number 1207836, Repeat Serious citation number 2, item number 1, and was affirmed as a final order on August 24, 2017, with respect to a workplace located at 31 Milliken Lane in Saint Augustine, Florida. Travis Slaughter d/b/a/ Great White construction, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.1053(b)(13), which was contained in OSHA inspection number 1207831, Repeat Serious citation number 2, item number 2, and was affirmed as a final order on August 24, 2017, with respect to a workplace located at 30 Milliken Lane in Saint Augustine, Florida. Travis Slaughter d/b/a/ Great White construction, Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.1053(b)(13), which was contained in OSHA inspection number 1147180, Repeat Serious citation number 1, item number 2, and was affirmed as a final order on October 11, 2016, with respect to a workplace located at 14609 Swansea Court, Jacksonville, Florida.
