29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: The employer is failing to protect employees from struck-by and engulfment hazards associated with trenching and excavations. This was most recently documented on or about January 11, 2019 at or near Stable Road, Saint Louis, MO 63110, employees were replacing a concrete storm drain pipe in an excavation measuring approximately 7.5 feet deep X 6 feet wide. The employer did not provide or require the use of benching, sloping, shoring or other means to ensure that employees working in excavations were protected from engulfment hazards.
