29 CFR 1910.269(p)(4)(i): The employer did not ensure that mechanical equipment was operated so that the minimum approach distances, established by the employer under paragraph (l)(3)(i) of this section, were maintained from exposed energized lines and equipment. On or about November 27, 2018, at a worksite located on St. Augustine Street, Dallas, Texas, employees working on replacing a pole near exposed energized electrical power lines were exposed to electrical shock and burn hazards.
