Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: ...the hazard of being struck by the end of a flying wire rope due to the failure under load of the end connection of the 3/8 inch wire rope used to haul the steel bar transfer car located in the center of the plant building. Among other methods, one feasible and acceptable abatement method to correct this hazard is to make up all wire rope end connections with the appropriate number of wire rope clips, to ensure that the saddle portion of the clips are installed on the long, or "live" end of the rope, and to properly torque the clip bolts, both at the time of installation, and periodically thereafter.
