29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) Construction site, Roof of unit block 13-25 - On or about October 4, 2018, after being instructed by the first tier contractor to properly rig their fall protection to limit excess slack, the employer permitted employees to repeatedly adjust their rope grabs to lengthen the rope in a manner that would allow free fall of more than 6 feet or contact a lower level.
