29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. Located at 37 Waterhill Street, Lynn, MA On or about September 26, 2018, the employer did not record a workplace injury on the OSHA 300 Form or equivalent for the calendar year 2018. On or about August 8, 2018, an employee received a partial finger amputation and it was not recorded on the OSHA 300 Form within seven calendar days.
