29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: The employer failed to report a work related hospitalization to OSHA within 24 hours of the incident occurring. This was most recently documented at 904 Deer Run Lane Papillion, Nebraska on or about May 29, 2018 when the employer had employees working on the roof of this residence when an employee of the company fell from a roof and was hospitalized. No notification of the hospitalization was made to OSHA by the employer.
