29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) Starch Department- On May 31, 2018, the employer failed to ensure that protective hardware, such as ball valve covers, were provided for use during lockout tagout to isolate the pneumatic energy on the Mogul Machines. b) Starch Department- On May 31, 2018, the employer failed to ensure that an adequate number of locks were provided to secure each energy source for use during lockout tagout. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
