29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k): (a) Handle Mill: On or about March 6, 2018 the employer had not provided training on the topics listed in 29 CFR 1910.95(k)(3)(i) through (k)(3)(iii) in that an employee (Chop Saw Operator) was exposed to continuous noise at 281.7 percent of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 480 minute sampling period on March 13, 2018. For a full eight hour work shift this exposure would be equivalent to an average sound level of 97.4 dBA. (b) Handle Mill: On or about March 6, 2018 the employer had not provided training on the topics listed in 29 CFR 1910.95(k)(3)(i) through (k)(3)(iii) in that an employee (Rip Saw Operator) was exposed to continuous noise at 164.7 percent of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 480 minute sampling period on March 13, 2018. For a full eight hour work shift this exposure would be equivalent to an average sound level of 93.5 dBA. (c) Handle Mill: On or about March 6, 2018 the employer had not provided training on the topics listed in 29 CFR 1910.95(k)(3)(i) through (k)(3)(iii) in that an employee (Dowel Milling Machine Operator) was exposed to continuous noise at 110.9 percent of the permissible daily noise exposure (8-hour Time Weighted Average sound level of 90 dBA) during the 480 minute sampling period on March 13, 2018. For a full eight hour work shift this exposure would be equivalent to an average sound level of 90.7 dBA. The Collins Tool Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95(k)(3)(iii), which was contained in OSHA inspection number 924665, citation number 1, item number 3c and was affirmed as a final order on January 2, 2014, with respect to a workplace located at 3254 Old Stage Road, Lewistown, PA 17044.
