29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: A) Major Metals Company, located at 800 Kochheiser Road, Mansfield, OH 44904: On or about February 8, 2018, the employer did not effectively train employees properly to lockout/tagout equipment such as, but not limited to, the Haven 3 tube cut off machine (Model # 872, Serial # 3020) located in Mill 4 Building. Employees did not control all hazardous energy sources such as, but not limited to, hydraulic before employees conducted activities such as, but not limited to, adjusting cooling lines located near the machines point of operation. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. Major Metals Company was previously cited for a violation of this Occupational Safety and Health Standard (1910.147(c)(7)(i)), which was contained in OSHA Inspection # 956027, Citation number 2, Item number 2C, and was affirmed as a final order on July 29, 2014, with respect to a workplace located at 844 Kochheiser Road, Mansfield, OH 44904.
