29 CFR 1904.29(b)(3): Each recordable injury or illness on the OSHA 300 Log and 301 Incident Report was not recorded within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: (a) Studio Theatre - On or about January 12, 2018, an employee received an injury to her ankle resulting in being placed on light duty for two weeks and the employer did not record it on the OSHA 300 log within seven calendar days for calendar year 2018. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
