29 CFR 1910.146(d)(5)(i): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not evaluate permit space conditions when entry operations were conducted by testing conditions in the permit space to determine if acceptable entry conditions exist before entry was authorized to begin: a. On or about 1/16/2018 at the establishment located at 5911 E. Dr. Martin Luther King Junior Blvd., Tampa, FL, for employees conducting permit-required confined space entry to clean chemical transporting tanks, a gas detector was not used to determine the atmospheric condition of the tank before entry to conduct cleaning activities, exposing employees to a hazardous atmosphere created by carbon monoxide. An employee lost consciousness while working inside the tank. The employee was exposed to 2,418.6 ppm of carbon monoxide (CO) over a 30 minute time period, based on the carboxyhemoglobin level in the employee's blood. This is equivalent to an 8-hour TWA exposure of 151.2 ppm, which is 3.02 times the OSHA-PEL of 50 ppm. The Tampa Bay Truck Wash, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1021462, citation number 1, item number 6 and was affirmed as a final order on 7/21/2015, with respect to a workplace located at 5911 E. Dr. Martin Luther King Junior Blvd., Tampa, FL.
