29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. Shipping/Receiving: On or about January 12, 2018, a forklift operator used a carpet needle attachment to move rolls of carpet. The Hyster sit down forklift (s/n - A268N16122N) was not marked to identify the carpet needle and show the weight of the truck and carpet needle combination at maximum elevation with load laterally centered not allowing operators to know the approximate weight capacities. Certification, with date and method of abatement required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within (10) calendar days after the abatement date.
