(a) Options Medical Center dba Options, 1700 S. 120th St., Lafayette, CO 80026: On or before January 25, 2018, the employer did not ensure that the area above the locker room could support the maximum intended load for that surface, in that the area was used to store several lighting units. This condition potentially exposed employees to struck-by hazards.
