29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about November 13, 2017 the employer failed to ensure that lockout devices were affixed to the hazardous energy isolating devices prior to removing a jam from the unimate robot located in the secondary machining area. An employee was observed performing the task by only using the stop button and entering a chain barrier guard. The TFP Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 1064884, citation number 1, item number 2b and was affirmed as a final order on July 28, 2015 with respect to a workplace located at 460 Lake Road, Medina, OH.
