Located at The Carlstar Group, LLC, McDonough, Georgia: On or about October 17, 2017, an incident Report (OSHA 301or equivalent) for each injury or illness was not filled out as required by the regulation. (a) On or about September 19, 2017, A 301 or equivalent was not filled out due when an employee sustained a back strain due to lifting a heavy tire and was transferred from his job duties for 16 days.
