29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: On or about October 17, 2017, employees were exposed to a trench cave in. Employees were involved in gas line repair at 3913 Darrow Road, Stow, Ohio 44224. An employee was working in a trench that was approximately 5 feet 5 inches deep without any form of cave in protection.
