29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: Employees were exposed to cave-in hazards while working in an excavation when entering and exiting a trench approximately 6-feet deep. 4 M Construction was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1160985, citation number 2, item number 1 and was affirmed as a final order on August 25, 2016, with respect to a workplace located at the 6700 Block of Broadway, Tyler, Texas.
