29 CFR 1926.502(d)(6)(v): Snap hooks shall not be engaged to any object which is incompatibly shaped or dimensioned in relation to the snap hook such that unintentional disengagement could occur by the connected object being able to depress the snap hook keeper and release itself: a) On or about September 14, 2017, an employee performing roofing operations in a hoist area engaged the snap hook of one vertical lifeline to the rope grab mechanism of a separate vertical lifeline to create one extended vertical lifeline.
