29 CFR 1910.29(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guiderail system and the walking-working surface: Mezzanine, Floor Hatch Area: On, or about, June 23, 2017, three sides of the railing system provided around the floor opening had the midrail installed 18 inches above the floor instead of approximately 21 inches, which would have been midway between the floor and top rail.
