29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. 1) On or about June 20, 2017, at the Tyson Prepared Foods facility at 9 N Washington in South Hutchinson, Kansas, the employer did not comply with recognized and generally accepted good engineering practice such as ANSI/ASHRAE 15, Section 8.11.2, when it did not ensure that doors were sealed to prevent releases of ammonia from the Compressor Room #1 to occupied spaces, exposing employees to the hazards of inhalation of toxic ammonia and ammonia burns. 2) On or about June 20, 2017, at the Tyson Prepared Foods facility at 9 N Washington in South Hutchinson, Kansas employees engaged in operating, inspecting, testing, and maintaining activities near the covered process equipment in each of the compressor rooms were exposed to the hazards of inhalation of toxic ammonia and ammonia burns leading to death and injury to persons in that the employer failed implement in compliance with recognized and generally accepted good engineering practices for the installation of emergency egress doors that would by design open outward from the compressor rooms to comply with IIAR 2-1992, ANSI/ASHRAE 15, and IIAR Bulletin 112-1998. The doors opening inwards impedes the employees egress from a potentially hazardous environment in the event of an ammonia release and were noted in the following areas: 2a) Compressor Room #1 2b) Compressor Room #2 2c) Compressor Room #3 2d) Compressor Room #4 Tyson Foods, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.119(d)(3)(ii), which was contained in OSHA inspection number 951317, citation number 1, item number 1 and was affirmed as a final order on 06/10/2014, with respect to a workplace located at 67240 Main Street, in Blountsville, Alabama.
