Text For Citation: 02 Item/Group: 002 Hazard:
29 CFR 1910.119(f)(1): The employer shall develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and shall address at least the following elements: 29 CFR 1910.119(f)(1)(ii): Operating limits: a. On or before July 13, 2017, in the employer's Booster Operating Procedures, LP-BX-002, Revision No. 35, Section 6: Safe Operating Limits, the safe operating limits for steam pressure was identified as 0 to 5 psig which did not distinguish between "operating limits" and "safe upper and lower limits" and was not consistent with the employer's PSI (process safety information) or other sections of the operating procedure which stated 6 psig maximum. b. On or before July 13, 2017, in the employer's Booster Operating Procedures, LP-BX-002, Revision No. 35, Section 6: Safe Operating Limits, the safe operating limits for the agitator RPM limit was identified as 22 RPM which did not distinguish between "operating limits" and "safe upper and lower limits" and was not consistent with the actual practice of operating at 27.5 RPMs and the employer's PSI which stated that the safe lower and upper limits for the agitator speed was 25 and 30 RPMs, respectively. c. On or before July 13, 2017, in the employer's Booster Operating Procedures, LP-BX-002, Revision No. 35, Section 6: Safe Operating Limits, the safe operating limits for the kettle temperature was identified as less than 200 degrees F which did not distinguish between "operating limits" and "safe upper and lower limits" and was not consistent with the employer's PSI which stated that the temperature was 228 degrees F for non-pentolite mixtures and 200 degrees F for pentolite mixtures. In addition, all of the consequences of deviation and steps required to correct or avoid deviation were not identified in the operating procedures as stated in the PSI. Austin Powder Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.119(f)(1)(ii), which was contained in OSHA inspection number 957166, citation number 1, item number 2b and was affirmed as a final order on August 21, 2014, with respect to a workplace located at 7LC10 Blandy Road, Camden, Arkansas 71701.
