29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every 3 years: (a) On or about May 30, 2017, the employer did not ensure that all employees who operated powered industrial trucks, were trained, evaluated and certified to perform the tasks in a safe manner at least once every three years. J.D. Graphic Co., Inc. was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 666518, citation number 1, item number 3 and was affirmed as a final order on October 30, 2012, with respect to a workplace located at 1101 Arthur Avenue, Elk Grove Village, IL 60007. No abatement certification or documentation is required for this item.
