29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): 101 North Main Street, Newark, Illinois, west building shop, powered industrial trucks (forklifts)- on or about May 4, 2017, employees were operating forklifts without undergoing initial workplace-specific formal and practical training, and evaluation in safe operation of the equipment. Operators and other employees working in the area were exposed to struck-by hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). Louis Dierzen dba Dierzen Sales, Ltd. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 1125876, citation number 01, item number 003 and was affirmed as a final order on August 29, 2016, with respect to a workplace located at 101 N. Main St., Newark, IL 60541.
